Required Documentation for Employment

  • W-4, Employee's Withholding Allowance Certificate, Department of the Treasury, Internal Revenue Service
    This form must be completed so your employer can withhold the correct Federal income tax from your pay.  Because your tax situation may change, you may want to refigure your withholding each year.

  • I-9, Employment Eligibility Verification, Department of Homeland Security
    The I-9 establishes the identity and employment eligibility of the prospective employee.   The form must be completed in the Human Resource Office and documents that establish identity and employment eligibility must be provided at that time.

  • Selective Service Registration
    The state law requires males between the ages of eighteen and twenty-six to provide proof of registration with the Selective Service prior to employment with the State of Texas and UT Permian Basin.  This is a condition of employment and must be provided to the Human Resource Office before the employee can work

Optional Employee Forms

  • Direct Deposit Authorization
    All employees of the University are eligible for the Direct Deposit of their payroll checks.  These forms are available in the Human Resource Office.
