The University of Texas of the Permian Basin (UTPB) provides access to its programs, classes, services, and activities to qualified individuals with disabilities as required by Section 504 of the Rehabilitation Act of 1973 and Title II of the Americans with Disabilities Act of 1990 as amended.

Under the ADA and Section 504, a person with a disability is defined as “any person who (1) has a physical or mental impairment that substantially limits one or more major life activities, (2) has a record of such impairment, or (3) is regarded as having such impairment.

U.T.P.B. encourages students with disabilities to visit the Testing Services & Academic Accommodations Department, where they may receive consultations and request reasonable accommodations (including in appropriate situations classroom adjustments/modifications) under Section 504 and the ADA.

To be considered for accommodations and/or services, a student must self-identify as having a disability and provide the Testing Services & Academic Accommodations Department with the required documentation of his/her disability at least one month before the accommodations and/or services are requested to begin. This information helps determine if a student is eligible for accommodations and, if so, for which specific accommodations.

Registration Procedures:

To request accommodations, you must register with the Testing Services & Academic Accommodations Department. This can take place once you have enrolled for the upcoming semester. If you are returning after two or more semesters of non-enrollment, you must re-register with this office. Current students must provide a current class schedule two weeks prior to the beginning of the semester; otherwise, letters of accommodations will not be issued.

How to Register:

  1. Come by in person to the Testing Services & Academic Accommodations Department, MB 1160, call at (432)552-2630, or visit our website at
  2. Make an initial intake appointment with the Disability Services designee.
  3. When making an initial appointment, students should briefly describe the nature of their disability. 
  4. At least 24 hours before the appointment, send any available documentation of the disability and, if applicable, a history of accommodations received in prior educational settings.
  5. Arrive 5 to 10 minutes prior to the scheduled appointment to fill out paperwork.
  6. Meet with the Disability Service designee to discuss eligibility for services and to identify academic, programmatic and/or physical accommodations that might be available.

Required Documentation:

All disability documentation mus be on letterhead signed by the appropriate licensed educational, mental health, or medical professional who is certified or licensed practitioner in the area for which the diagnosis is made. NOTE: Documentation presented on a prescription pad slip by itself, is unlikely to be sufficient.

In addition, all documents should provide the following information:

  • Date of the evaluation
  • Method of evaluation/examination
  • Specific diagnosis
  • Medication – expected effects on functioning, and any side effects
  • Listing and discussion of specific limitations with respect to the disability’s impact in academic and academic related environments as it relates to the accommodations requested.

Finally, please note that documentation should not be more than five (5) years old.

Students who provide all documentation and are eligible for services will be formally approved during an intake interview with the disability services office. Accommodations will be determined on a case-by-case basis, based on the documentation, the student’s history and specific functional limitations. Students requesting accommodations in the classroom will be provided with a letter prepared by the Testing Services & Academic Accommodations Department verifying the need for accommodations to each of their instructors.


If a student is eligible to receive accommodations, the accommodations will be determined on a case-by-case basis, based on the documentation, the student’s history, specific functional limitations, and other information. Students approved to receive accommodations in the classroom will be provided a letter prepared by the Testing Services & Academic Accommodations Department; that letter lists the approved accommodations, and the student should present it to all instructors in which accommodation is desired.

Depending on the situation, the following accommodations and others may be available to qualifying students with disabilities at U.T.P.B.:

  • Note takers
  • Extra time for test
  • Isolated testing rooms
  • Enlarged print on computers
