Please turn in this completed form and a copy of the receipt for payment of course fees in the Continuing Education Office on the fourth floor of UTPB MESA Building in room MB 4207. Fees are to be paid by cash or credit card in UTPB Financial Services office on the first floor of the UTPB MESA Building in room MB 1220.
