General Financial Aid Application (Fall & Spring) - This is an institutional application that may be completed to help determine your eligibility for the semester(s) indicated at the top of the form. This application can be completed annually for the Fall and Spring semesters. This form is valid only for the semester(s) noted at the top of the form.
U.T. Permian Basin Scholarship Application U.T. Permian Basin scholarships are available for both graduate and undergraduate students Students may apply for scholarships by completing the General Financial Aid Application. The applications are available in November of each year.
It also is recommended that scholarship applicants complete the Free Application for Federal Student Aid (FAFSA). The amount of scholarship and other financial aid cannot exceed that provided for in-state and federal regulations or, in any case, exceed the estimated cost per year of student expenses as determined by U.T. Permian Basin's Office of Financial Aid. Recipients of scholarships awarded by U.T. Permian Basin must be enrolled as full-time students (12 semester hours for an undergraduate or 9 semester hours for a graduate student). Work Study Student Employment Application - The information collected on this form will be used to evaluate student employment opportunities. Positions are available on a first-come, first-serve basis. Completing this application does not guarantee a position. This form is valid only for the semester(s) noted at the top of the form.
Request for Special Circumstance - This form is for a professional judgment. It is used if there has been a negative change in your or your family's financial situation because of one of the following:
- Loss of Job or Benefits.
- Separation or Divorce.
- Loss Because of Death.
Unusual medical or elementary education expenses paid by you or your parents, and not reimbursable through any agency.
In addition to the required documents, you must submit a letter describing your special circumstances in detail. A request without required documentation will be rejected. You must complete the FASFA using actual data. The Office of Student Financial Aid will print you SAR Failure to make an appointment may result in your request being rejected. If funds have been disbursed to you or your student account before completing this process, your request will be rejected. The information collected on this application will be used to determine if a special circumstance is appropriate. A change is at the discretion of the Assistant Director of Student Financial Aid. There are no appeals. Additional documents may be required in addition to this application. YOU MUST MAKE AN APPOINTMENT WITH THE DIRECTOR OF FINANCIAL AID BEFORE THIS REQUEST CAN BE PROCESSED. Hazlewood Veterans Exemption - This form is used to verify Hazlewood Exemptions. It must be submitted each semester that the student plans to use it. The student will also be required to furnish the following:
- Proof of Texas State Residency
- Proof of Honorable Discharge (Copy of DD-214 form should state this)
- Proof of expiration of VA Educational Benefits (Call 888-442-4551)
- Copy of DD-214

Request for Dependent Care - This form is for a professional judgment. The information collected on this application will be used to determine if a special circumstance is appropriate. A change is at the discretion of the Assistant Director of Student Financial Aid. There are no appeals. The information on this form is used to validate dependent care expenses for audit purposes. Disclosure Authorization - It is the policy of the Office of Student Financial Aid to release specific information regarding a student's financial aid only to the student.
