Hazlewood Exemption

To apply for the Hazlewood Exemption, applicants must:

• be in a degree-seeking program
• be enrolled in state-funded courses
• meet Satisfactory Academic Progress (SAP) as determined by the Office of Financial Aid & Scholarships, and 
• comply with Texas Education Code §54.2001 as determined by the Student Accounts Office.

The application deadline, including all supporting documents, is 5:00 pm of the last class day, as published in the academic calendar, for the semester or term to which the exemption applies. 

We recommend that you complete and submit your application at least 4 weeks before the tuition deadline so the veteran certification team can process your application before tuition is due. Previous eligibility determinations by other institutions DO NOT require UT Permian Basin to automatically award the exemption. Eligibility will only be determined by applying and submitting the required documentation.

Veterans using the Exemption:

• Texas Hazlewood Exemption Application
• Certificate of Separation or Discharge (DD Form 214) – The copy you provide must be legible and contain place of entry, home of record at the time of entry, character of service, length of service, and dates of service (Service 2 or Member4).
• Certificate of Eligibility for Federal GI Bill^® benefits (COE) – Required for veterans who served after September 11, 2001.
• Registration with the Hazlewood Student Hours Online database – Once registered, log in and print the Account Overview page (even if hours show zero) for the school.
• Proof of Residency – Veterans must reside in the state of Texas during the term the exemption is used.

Legacy Act – Veterans Transferring Unused Hours to an Eligible Child:

• Texas Hazlewood Exemption Application
• Certificate of Separation or Discharge (DD Form 214) – The copy you provide must be legible and contain place of entry, home of record at the time of entry, character of service, length of service, and dates of service (Service 2 or Member4).
• Certificate of Eligibility for Federal GI Bill^® benefits (COE) – Required for veterans who served after September 11, 2001.
• Relationship Documentation – a copy of the student’s birth certificate, adoption papers, or a copy of the veteran’s most recent tax return showing the student claimed as a dependent.
• Registration with the Hazlewood Student Hours Online database – Once registered, log in and print the Account Overview page (even if hours show zero) for the school.
• Proof of Residency – Veterans must reside in the state of Texas during the term the exemption is used. Children and spouse of eligible Veterans must be classified as a Texas Resident by the institution.

Spouse/Child of 100% Disabled or Deceased Veteran:

• Texas Hazlewood Exemption Application
• Certificate of Separation or Discharge (DD Form 214) – The copy you provide must be legible and contain place of entry, home of record at the time of entry, character of service, length of service, and dates of service (Service 2 or Member4).
• Documentation from DOD or VA of a veteran’s service-related death or 100 percent disability
• Relationship Documentation – a copy of the student’s birth certificate, adoption papers, or a copy of the veteran’s most recent tax return showing the student claimed as a dependent.
• Registration with the Hazlewood Student Hours Online database – Once registered, log in and print the Account Overview page (even if hours show zero) for the school.
• Proof of Residency – Veterans must reside in the state of Texas during the term the exemption is used. Children and spouse of eligible Veterans must be classified as a Texas Resident by the institution.

The initial Hazlewood Applications must be resubmitted each academic year, beginning with the fall term.

Supporting documentation need not be resubmitted annually unless there has been a change in VA benefits status.
