The University of Texas Permian Basin is committed to creating a safe, supportive learning and working environment for all members of the campus community. Campus safety is our top priority, and we take all reports of misconduct seriously to protect everyone’s health and well-being.
Anyone can submit a report using the links below. Students and other members of the campus community are encouraged to share the concerns they have through the appropriate report below. However, reporters should understand that UTPB takes all reports seriously, and that students knowingly making a false report may be reported to the Office of the Dean of Students.

If you see something, say and do something about it—report any concerning behavior or suspicious activity using the forms below. Using any form below will route the information to the Office of the Dean of Students, the important thing is to make the report it!
Emergency: Situations that present an immediate threat to persons or property are considered emergencies and should be directed to the UTPB Police Department at 432-552-2911.
Report Student Misconduct
This form can be used to report information about student misconduct or behavior that could potentially violate policies at UT Permian Basin. This form can be used for individuals and/or organizations.
Report Academic Dishonesty
Scholastic Dishonesty is any form of cheating or plagiarism that violates the Student Code of Conduct. Scholastic dishonesty or academic dishonesty includes, but is not limited to cheating, plagiarism, collusion, falsifying academic records, and any act designed to give unfair advantage to the student, or the attempt to commit such an act. If you have completed the faculty disposition of the scholastic dishonesty policy form, please upload this form in the supporting documents section of the form below.

Faculty, staff, and students are encouraged to use this form to report concerning behavior that may be impacting a student’s well-being or academic success.

Report Sexual Misconduct
UT Permian Basin is committed to the principle that the University’s working and learning environment be free from inappropriate conduct of a sexual nature. Sexual harassment or
sexual misconduct in any form will not be tolerated and individuals who engage in such conduct are subject to disciplinary action.
Report Unlawful Harassment, Discrimination or Retaliation
To the extent provided by applicable law, no person shall be excluded from participation in, denied the benefits of, or be subject to discrimination under, any program or activity sponsored or conducted by UT Permian Basin, on the basis of race, color, national origin, religion, sex, age, veteran status, or disability.

The Academic Early Alert Report is intended to connect students who are having difficulty with reading comprehension, class assignments, class attendance, class participation, or writing to campus resources such as tutoring, peer mentoring, Student Success Center, and other academic support resources.
